Your business tax questionnaire

What the business questionnaire asks — entity and formation docs, ownership, employees vs. contractors, state filings, and QuickBooks for tax-only clients.

Your business tax questionnaire

If Uplinq files for your business, you'll complete a business tax questionnaire. It's a smart questionnaire, not a flat checklist: as you answer, your specific answers decide which documents we ask you to upload. That means you only get asked for the things that actually apply to your entity.

The one rule that explains everything

Answer a question a certain way, and Uplinq automatically opens a matching document request for you. Answer X → we ask you to upload document Y. There's no master list — your requests are built from your answers, and they appear in your File Requests tab as you go.

How your answers turn into document requests

The questionnaire moves through your business section by section. When an answer means a document exists, we open a request for it — usually in real time. A few patterns to expect:

  • A Yes can trigger a request (you file taxes in Texas → we ask for your Texas Webfile ID).
  • A No can too, when it means foundational paperwork is still outstanding (you haven't filed before → we ask for your formation documents).
  • Your ownership structure changes what we need (single-member vs. multiple owners).

The sections below are the ones that most often generate requests.

State filings

Where you file changes what we need. Texas is a common example.

When you answer……we'll typically request
You file taxes in TexasTexas Webfile ID
Other states, same idea

The questionnaire also covers state apportionment when you operate in more than one state. The principle is identical — the states you tell us about decide which state-specific items we ask for.

Entity & formation documents

If this is your first time filing with us, we need the paperwork that establishes the business.

When you answer……we'll typically request
You haven't filed beforeArticles of Organization (or Incorporation) + EIN letter + equity cap table
Ownership is a single memberArticles of Organization (LLC)
Ownership is multiple ownersequity cap table

Employees vs. contractors

How you pay your people decides which forms we collect — and whether we're filing them or you are.

When you answer……we'll typically request
You have employeesEmployee(s) W-2s
You use contractors and filed the 1099s yourselfContractor(s) 1099s
You use contractors and want Uplinq to fileContractor W-9s

Tax-only clients & QuickBooks

If we handle your taxes but not your day-to-day bookkeeping, we need a way to get your financials.

When you answer……we'll typically request
You're tax-only, financials are in QuickBooks (and you'll grant access)QBO Credentials for Tax Only
You're tax-only and not in QuickBooks / can't grant accessPrior-Year Financial Reports
There are more sections than these

The questionnaire also covers business-structure changes, deduction configurations (retirement, medical, life), personally-paid expenses, assets, and auto expenses, plus general file requests. Everywhere, the same rule holds: your answer decides what we ask for.

If a document doesn't apply to you

Answer each question as it's true for your business and no request is created. If a request does appear for something you genuinely don't have — say, an operating agreement when you're a single member — leave a short "N/A" comment so we know it isn't missing. See Uploading files & File Requests.
