Your business tax questionnaire
What the business questionnaire asks — entity and formation docs, ownership, employees vs. contractors, state filings, and QuickBooks for tax-only clients.
If Uplinq files for your business, you'll complete a business tax questionnaire. It's a smart questionnaire, not a flat checklist: your answers help us decide which documents to request and which details your return needs.
Your answers guide your document requests. Some requests may appear while you answer, and others may come after Uplinq reviews your questionnaire.
If you know a document applies to you or something looks missing, ask your tax contact before assuming no request means no document is needed.
How your answers turn into document requests
The questionnaire moves through your business section by section. When an answer points to a document we need, Uplinq may create a request for it. A few patterns to expect:
- A Yes can trigger a request (you file taxes in Texas → we ask for your Texas Webfile ID).
- A No can too, when it means foundational paperwork is still outstanding (you haven't filed before → we ask for your formation documents).
- Your ownership structure changes what we need (single-member vs. multiple owners).
The sections below are the ones that most often generate requests.
State filings

The questionnaire also covers state apportionment when you operate in more than one state. The principle is identical — the states you tell us about decide which state-specific items we ask for.
Entity & formation documents
If this is your first time filing with us, we need the paperwork that establishes the business.

Employees vs. contractors
How you pay your people decides which forms we collect — and whether we're filing them or you are.

Tax-only clients & QuickBooks
If we handle your taxes but not your day-to-day bookkeeping, we need a way to get your financials.

The questionnaire also covers business-structure changes, deduction configurations (retirement, medical, life), personally-paid expenses, assets, and auto expenses, plus general file requests. Your answers are the starting point, and your tax contact may ask for more if review shows another document is needed.
Answer each question as it's true for your business. If a request appears for something you genuinely don't have, leave a short "N/A" comment so we know it isn't missing.
